Basic Life and AD &D Insurance Benefits:

The Life plan provided by Aetna has a basic death benefit of $10,000 for all eligible full time employees that is payable to designated beneficiary(s) in the event of death.

The AD&D plan provided by Aetna will pay a principal sum of basic earnings to a maximum of $10,000 for the loss of use of limbs due to an accident.

Basic Long Term Disability Benefits

This long term disability plan provided by Aetna will pay a monthly benefit for a period of total disability caused by disease or accidental bodily injury.  Upon a waiting period of 180 days the maximum monthly benefit of $2,000 will begin.
